While Blizzard has provided some information on the conditions for obtaining these items, players have been left to determine the actual drop rates through their own efforts, leading to perplexity and dissatisfaction.
A Diablo 4 developer clarified that the six Unique items—Doombringer, The Grandfather, Ring of Starless Skies, Andariel's Visage, Harlequin Crest, and Melted Heart of Selig—only drop from enemies of level 85 and above. These items are categorized as "realllly rare," boasting significantly lower drop rates than other Uniques. However, the precise rarity level has not been officially disclosed.

The challenge lies in the fact that loot drop rate information is stored server-side, preventing players from datamining it directly. Instead, some players have loosely estimated the number of item drops reported online relative to the total hours played in Diablo 4—a figure recently confirmed by Blizzard.
